• Shift: Days | 5x8-Hour (08:00 AM – 4:30 PM)
• Start Dates: 09/02, 09/08, 09/15

Requirements:

• Minimum 2 years of recent acute care hospital Case Manager experience
• Strong discharge planning and interdisciplinary communication skills
• Proficiency with Epic
